We have a team of Nurses, CNA’s, PCA’s LPN’s, who can be assigned to help your loved one at home with health management. Nursing Care starts with an assessment of your health care needs a by one of our Registered Nurse and the outline of an initial care plan.
After the care plan is reviewed, our professionals may be assigned with the following health management functions:
- Disease Management and Symptom Control
- Nutrition Monitoring
- Post Treatment Home Care Needs
- Taking and Monitoring Patient’s Vital Signs
- Safe Mobility Support for the Patient
- Self Administration of medication
